Dr. David Christopher Fuller, PT, DPT

Chris, as the CEO of Fuller Physical Therapy, graduated from Bellarmine University with his Doctorate of Physical Therapy (DPT) in 2005. He recently acquired his Masters in business (MBA) from Western Kentucky University. Chris specializes in manual therapy treatment for the spine and extremities, trigger point dry needling, temporomandibular joint (TMJ) dysfunctions and pain.

He lives in Hartford, KY with his wife and 2 sons and is active in his church and enjoys hunting, fishing, and four wheeling.

David Philip Embry, PT

Philip Embry is a 1996 graduate from the University of Louisville school of Health and Allied Science with a bachelor degree in Physical Therapy. Philip has a wide range of physical therapy experiences in pediatrics, geriatrics, orthopedics, neurological conditions, and treatment of many other physical impairments.

Philip is a graduate of Ohio County High School and has been a proud resident of Ohio County for his entire life. Philip has been married to his loving wife, Angie, for over 18 years and is blessed with two children; River, 16, and Rain, 11. Philip is also an ordained minister at a local church and enjoys hunting and fishing as well as tending to his farm.

Kayla Kukahiko, PT, DPT

Kayla is a 2012 graduate from the University of Kentucky with her Doctorate of Physical Therapy. Kayla has had past experiences as supervisor of an outpatient clinic, as well as, inpatient and skilled nursing rehabilitation. Kayla is from Hanover, IN and resides in Princeton, KY. Kayla is married to Daniel, who teaches and coaches, and together they have a daughter, Hadley. Kayla enjoys spending time with her family, being active in her church, serving as a member of Vision West Kentucky, enjoying the outdoors, attending her husband’s basketball games, and cheering for the Hoosiers.

Casie Stivers PT, DPT

Casie is a 2016 graduate from Belmont University’s Doctorate of Physical Therapy program. In addition to her Doctorate in Physical Therapy, she also holds two bachelor’s degrees from Western KY University in Biology and Exercise Science. Casie has been employed with Fuller Physical Therapy since Oct. 2016. She has varied clinical experience in treating most orthopedic and neurologic conditions. She is passionate in treating patients who complain of “dizziness” or vestibular dysfunction. Casie also holds a certification in trigger point dry needling and utilizes this technique to assist in treating a whole host of musculoskeletal disorders. She also enjoys treating athletic injuries and providing education on injury prevention. She is looking to further her clinical education by pursing an orthopedic manual therapy and vestibular rehabilitation certification in the near future.

Casie is married to Jesse Stivers and they currently live in Morgantown, KY. They enjoy attending their church (Belmont), remaining physically active through running and CrossFit, and spending time with their family and friends. Casie is a Butler County native and is excited to use her skills to serve the community in which she grew up!

Brittany Phelps, PT, DPT

Brittany graduated from Bellarmine University’s Doctor of Physical Therapy program in May, 2017. She completed her undergraduate coursework at Western Kentucky University in 2014. Before graduating from the Doctoral program, Brittany enjoyed providing dance therapy in a Parkinson’s Dance Class, and other pro bono PT services in Hobb’s Outpatient Clinic located on campus. Also during graduate school, Brittany completed an internship here at Fuller Physical Therapy and is very excited to continue serving the community near her home town. Her passion in physical therapy is to educate patients on pain management, and help them return to enjoying life.

Brittany grew up in Horse Branch, but now resides in Beaver Dam, Kentucky. In her free time, she and her husband Zach enjoy all things outdoors, including hunting and 4-wheeling.

Brian Belcher, PTA, BS, CI

Brian attended Madisonville Community College where he graduated with Distinction in 2014. He is the recipient of the 2014 MCC Outstanding PTA Student Award and a finalist for the 2014 MCC Gardner Award. He is a 2007 Graduate of Western Kentucky University where he earned his Bachelor's degree in Exercise Science. He has been an active member of the American Physical Therapy Association (APTA) and Kentucky Physical Therapy Association (KPTA) since 2013. Brian is an APTA Credentialed Clinical Instructor. He is currently pursuing an advanced proficiency credential in the area of Orthopedics.

Brian is married and has two daughters. In his free time he enjoys traveling, playing tennis, and spending time with his family. Brian treats patients at Fuller Physical Therapy- Beaver Dam.

Kristen Gary-Brooks, PTA

Kristen attended Madisonville Community College where she graduated with distinction in May, 2017. She was awarded Leader of Professional Excellence for 2017 after completing a professional portfolio consisting of employment documents, reflection pieces of time spent on campus and community service, professional development, networking activities and recommendations from those who can attest to her professionalism. Kristen has had past experiences in outpatient, inpatient and skilled nursing rehabilitation. Her areas of interest are orthopedics, neurology and pain management.

She is a 2007 graduate of Butler County High School and has been a proud resident of Butler County all of her life. Kristen enjoys the outdoors, travelling and making memories with friends and family.
